Base Plan: No cost to you for you and your family
100% premium paid by Gold Medal
$500 deductible/person $1000/family
Rx $100 deductible per yr per/person then # tier co-pay $10, $25, $45

Buy-Up Plan: $7.50/wk(pre tax) single
$15.00/wk(pre tax) family
Portion of premium paid by Gold Medal
$250 deductible/person $500 deductible/family
Rx No deductible 3 tier co-pay $10, $20, $30

*If eligible, your spouse must carry insurance through own employer.

Dental Coverage - Dental Care Plus

Employee is eligible the first of the month following 90 days. Gold Medal pays $5.00. Employee cost (can be paid pre tax) is as follows:

Base Plan: Single $11.68/mon, Double $27.61/mon, Family $49.98/mon

Buy up Plan: Double $29.03/mon, Family $52.41/mon
Buy-Up Plan includes orthodontics for kids age 18 & under

401(k) Plan - Administered by Fifth Third Bank

Can contribute after 90 days as much of gross income as desired.
Gold Medal Products Co. matches 72% of first 12%
100 % vested January 1 following 3 years of service

Life Insurance: Principal Financial Group

$10,000.00 policy after 90 days
2x W-2 (in previous year) policy after 2 years of employment

Vacation/Personal time off

Time given out January 2 of every year

Full years 1-4: 10 vacation days, 5 personal days
Full years 5-9: 10 vacation days, 7 personal days
Full years 10-19: 15 vacation days, 7 personal days
Full years 20+: 20 vacation days, 7 personal days
For first non-full year, time off is based on hours worked
Time off is carried over to following year if not used
Time off can be cashed in for pay

Major Holidays off with pay:

New Year's Day (floating), Thanksgiving Day, Memorial Day, Day after Thanksgiving, Fourth of July, Christmas Eve (floating), Labor Day, and Christmas Day (floating)

Tuition Reimbursement

Eligible, pre-approved classes for reimbursement up to $1500.00 / year for tuition and books.
