Employee is eligible the first of the month following 90 days. Gold Medal pays $5.00. Employee cost (can be paid pre tax) is as follows:
Base Plan: Single $11.68/mon, Double $27.61/mon, Family $49.98/mon
Buy up Plan: Double $29.03/mon, Family $52.41/mon
Buy-Up Plan includes orthodontics for kids age 18 & under
Can contribute after 90 days as much of gross income as desired.

100 % vested January 1 following 3 years of service
$10,000.00 policy after 90 days
2x W-2 (in previous year) policy after 2 years of employment
Time given out January 2 of every year
Full years 1-4: 10 vacation days, 5 personal days
Full years 5-9: 10 vacation days, 7 personal days
Full years 10-19: 15 vacation days, 7 personal days
Full years 20+: 20 vacation days, 7 personal days
For first non-full year, time off is based on hours worked
Time off is carried over to following year if not used
Time off can be cashed in for pay
New Year's Day (floating), Thanksgiving Day, Memorial Day, Day after Thanksgiving, Fourth of July, Christmas Eve (floating), Labor Day, and Christmas Day (floating)
Eligible, pre-approved classes for reimbursement up to $1500.00 / year for tuition and books.
